We are carefully monitoring the progression of the COVID-19 coronavirus.  As always, the safety and health of our patients, clients, and employees is our utmost concern.

Symptoms of COVID-19 include: fever, cough, and shortness of breath. Some people may have only mild symptoms, but they can still pass the virus to others who could have more severe symptoms.

If you have a cough, cold, fever, or flu symptoms, it is important to stay home and help prevent the spread of germs.

New Patients: Please be advised, Equitas Health providers do not prescribe controlled substances for new patients nor do we manage chronic pain issues. If you are experiencing chronic pain problems, your Equitas Health provider will refer you to a specialist outside of our system or will help you explore alternative methods of managing pain.

After Hours Care

If this is an emergency, please call 9-1-1 or go to the nearest emergency room.

For non-urgent issues, please call one of our medical centers. All after hours calls are answered by an on-call medical professional that will help answer your questions.

Scheduling: Please use MyChart to schedule appointments and communicate with your provider. You may also make an appointment by calling your preferred Equitas Health location.

You can also submit a records request in person at any of our medical centers. Our front desk staff will be happy to assist you with your needs.

Equitas Health will provide access to the patient health information (PHI) requested in less than 30 business days from receiving the individual’s request. We realize there are circumstances when you may need your medical record more urgently. If this is your situation, please call the Medical Records Department to discuss your needs.

Fees: There may be a fee for medical records, we will notify you of a potential fee in advance. There is no fee for copies requested by healthcare providers for continued care.
